Quotes, estimates and bookings
Prices, estimates and availability shown online are indicative until confirmed in writing by our team. Final pricing depends on an assessment of your specific needs. Bookings are confirmed only when you receive a confirmation from us.
Cancellations
Please give us as much notice as possible if you need to cancel or reschedule. Any applicable cancellation terms will be explained when your booking is confirmed.
Limitation of liability
To the fullest extent permitted by law, we are not liable for indirect or consequential losses arising from use of this website. Nothing in these terms limits liability that cannot be limited by law.

All content on this website, including the care level navigator, the screening schedule, the immunisation timeline, preparation guidance, laboratory turnaround information and the journal articles, is general health information published for education. It is not medical advice, it is not a diagnosis, and it does not take account of your personal or family history, your medications or your circumstances.
Using this website, submitting a form, or reading an article does not create a physician-patient relationship with Aurelia Concierge Medicine or with any clinician named on this site. That relationship begins only when a clinician accepts you as a patient and you complete the consent to treat.

Emergencies and urgent care
No form, message or email on this website is monitored for emergencies. If this is a medical emergency, call 911. If you are having thoughts of harming yourself, call or text 988 for the Suicide and Crisis Lifeline, or call 911. Members with an urgent but non-emergency question should call the 24 hour member line on (310) 555-0177, where a physician from the practice answers.
Membership is not health insurance
Membership at Aurelia is a service fee paid directly to the practice for access to and delivery of primary care. It is not health insurance, it is not a health plan, it is not a discount health plan, and it does not satisfy any legal requirement to hold coverage.
Membership does not pay for hospital admission, surgery, emergency care, specialist consultations, imaging performed outside our clinics, laboratory send-out testing, prescriptions or any care delivered by another provider. We strongly recommend that every member maintains separate health insurance.
We are a direct-pay practice and do not bill health plans for membership. We provide an itemised superbill after each visit so that you may seek out-of-network reimbursement. Whether your plan reimburses, and at what rate, is a matter between you and your insurer.
Telehealth terms
Telehealth visits are available only where the clinician holds a licence covering your physical location at the time of the visit. Our clinicians are licensed in California. A video visit has real clinical limitations: the clinician cannot palpate, auscultate, perform otoscopy, take a specimen or examine a joint, and a condition may be missed that an in-person examination would have found.
Prescribing by telehealth is subject to federal and California law, and controlled substances are subject to additional requirements that may make an in-person visit necessary first. By joining a video visit you consent to these limitations and to the recording of the clinical note in your record. You may withdraw consent and request an in-person visit at any time.
Appointment requests, pricing and sample content
Submitting an appointment request on this website does not create a confirmed appointment. A coordinator will contact you to confirm a date, a time and a clinician.
