Dr. Nadia Rahimi spent eleven years in a large Los Angeles internal medicine group. By the end she carried more than 2,200 patients and had twelve minutes per appointment. She has written since that the problem was never the clinicians inside that system; it was the arithmetic they were handed.
Twelve minutes is enough to address one complaint. It is not enough to reconcile nine medications from four prescribers, take a family history that goes past the word cancer, or ask a second question after the first answer sounds wrong. Those are the three places where primary care earns its keep.
Aurelia opened in 2016 with forty members and a cap. The cap is the whole design. Same-day access, 45-minute visits and a physician who answers the phone at 2 am are not features bolted onto a practice; they are what becomes possible when one physician looks after 240 people instead of 2,200.
The trade-off is honest and worth stating: this model works because members pay a fee outside insurance, and that is not available to everyone. We do not claim it is a solution to primary care in the United States. It is a solution for the people who join.

A 45-minute visit is not a luxury. It is how a medication list gets reconciled, how a family history gets taken properly, and how a diagnosis stops being a guess.
We request everything, read it, and resolve the contradictions. Most new members arrive with four records that disagree about their medication.
We are primary care. We do not perform surgery, we are not an emergency department, and membership is not insurance. Saying so clearly is part of the service.
An ApoB target, a screening interval, a date. Advice without a measurable target is a conversation, not a plan.

2019
Dr. Elliot Vance joins and ApoB and lipoprotein(a) become part of the standard baseline panel.

A second clinic built around women's health and sleep medicine, with longer appointment blocks by design.
2023
The one-day physical moves into its own suite so the whole sequence can run without leaving the building.

Panels capped, a waiting list when they are full, and the Palisades consulting rooms open three days a week.
